Why Rhinoplasty surgeries?
Rhinoplasty (Nose Job):
The face is the most exposed part of our body and the nose is located in the center and is prominent, so the structural defects of the nose are clearly visible to all people. Rhinoplasty and cosmetic surgery, in addition to creating a fit in the nose itself, establish a more pleasant connection between the nose and other parts of the face. For this reason, the nose should not be considered as a separate and single organ. The relationship of the nose to the forehead, eyes, lips and chin is such that any defect in the parts of the face can affect this delicate relationship. For example, a small chin makes the nose appear more prominent. Ignoring the fit of the nose with the components of the face will lead to a lack of satisfaction with the result.
Nowadays Rhinoplasty (also known as: nose job), is one of the surgical procedures that is widely used by many people around the world to reshape their nose. It is worth to know that the nose is one of the most distinctive features of your face and its function is critical to your beauty. Many people use Rhinoplasty because they aren’t satisfied with their nose’s appearance while others do it due to breathing difficulties.
Rhinoplasty is not an emergency surgery, on the other hand, the most difficult plastic surgery in the body is Rhinoplasty because we are dealing with an organ with complex vital functions. Therefore, patients should minimize the possibility of complications after careful examination and research and spending time choosing a surgeon.
Breathing through the nose is extremely important for our health. The nose is not just a conduit for the passage of inhaled air, but the nose, while passing the inhaled air, makes it completely clean, warm and moist. Obviously, nasal obstruction deprives a person of these benefits.
Paying attention to two characteristics, namely the importance of breathing through the nose and the amazing effect of the appearance of the nose on a person’s face, indicates that in addition to having the necessary knowledge, the Rhinosurgeon must have sufficient experience to be able to maintain nasal function and reach the nose. Balance more beautifully. In other words, the experienced surgeon will consider before making any changes to the appearance of the nose that this change does not impair the function of the nose and block the nose.
In the not-so-distant past, Rhinoplasty was limited to shrinking and shrinking the nose, and it was accepted that a nose was small, high, arched, and so-called puppet, but today, the science of Rhinoplasty has undergone a dramatic change. “Enhancing Rhinoplasty” is eliminating “traditional Rhinoplasty”.
In augmentation Rhinoplasty, the surgery is not limited to shrinking the nose, but in this method, the surgeon, while removing and shrinking parts of the nose, may strengthen other parts to be the end result of the nose, which while beautified but in function. The main thing is the same as breathing is not harmed. It is clear that such a nose is no longer too small and high and will not have the appearance of an extremely operated nose.
By observing the above, the possibility of complications following Rhinoplasty will be greatly reduced. Of course, a surgeon can predict by examining the nose that a particular nose may have a specific complication if it is operated on. In this case, the surgeon should discuss this with the applicant and try to dissuade him from performing the operation.
Rhinoplasty Treatment:
Rhinoplasty is one of the best cosmetic surgery to create the nose you have always desired.

The possible side effects: temporary swelling, bruising around the eyes or nose, bleeding, nasal stuffiness.
The risks: Infection, Asymmetry, unsatisfactory nasal shape .incomplete improvement that means additional surgery would be required.

The following various steps may be part of a Rhinoplasty procedure:
1- Correction of anomaly and deviation of nasal septum (Septoplasty)
2- Removal of bony and cartilaginous hump.
3- Repairing the tip of the nose, such as narrowing and raising.
4- Breaking and cutting the nasal bones to narrow the upper and middle part of the nose.
5- Addition of bone, bone and cartilage grafts in cases of weak support, nasal skeleton or deformities such as nasopharyngeal.
- Sewing skin and plastering.
- In closed surgery, the cuts are completely inside the nose. Open surgery is usually longer in duration than closed surgery and is best suited for patients who need to change the tips of their nose. Closed surgery avoids a clear cut on the nose, but this method cannot be used in cases where there is a need for significant changes to the nose. pen Rhinoplasty and closed Rhinoplasty surgery are different and each has its own traits, so it can’t be said for sure that one is superior to another. One of the advantages of open Rhinoplasty is that it enables the surgeon to resolve nose problems easily and make various changes. But in case of closed Rhinoplasty, surgeon can only remove nose hump and it might not eliminate the problem completely. Many doctors who perform Rhinoplasty prefer the open procedure, unless patient is dealing with various health problems.
- In open surgery, the tissue between the two nostrils named Columella is cut, the columella is a cartilage-like blade located at the bottom of the nasal septum that separates the two left and right nostrils. The columella is a wall of nasal tissue and connecting the tip of the nose to the nasal base. This wall forms the base of the nose. This procedure must be performed by doctors specialized in ear-throat-nose field (ENT) or plastic surgery or Maxillofacial surgery. Because only these specialist has the needed expertise to perform this surgical procedure. In this method, an incision is made across the narrow strip of tissue that separates the nostril.
The only advantage of closed nasal surgery is the absence of incision in the middle of the nose, but the accuracy and outcome of nasal surgery with the open technique are greater and it is effective in correcting nasal aberration.
Nose types in terms of Rhinoplasty
1) Humped Nose
2) Crooked or Asymmetrical Nose

What are the steps involved in Rhinoplasty surgery?
Stages of Rhinoplasty operation: Rhinoplasty is a correction, surgical, structural, external, nasal surgery to correct defects, beauty, or functional, congenital, and acquired Rhinoplasty. Patients are first asked about unilateral or bilateral nasal congestion, problems with the sinuses around the nose, trauma to the nose, or a history of anterior nasal surgery. The patient’s opinions about the appearance of his nose are evaluated. All indoor and outdoor buildings are inspected.
From the profile view, the fit of the nose with other parts of the face is examined. The upper third of the nose is bony and the lower two thirds is cartilaginous. In profile view, the protrusion of the back of the nose and the drooping of the tip of the nose are examined.
In general, Rhinoplasty is the access to the skeletal, skeletal, and cartilaginous skeleton through an incision inside or outside the nose, based on the location of the deformity. Bone and cartilaginous humps can be adjusted to change the size and shape of the tip of the nose.
Various nasal defects can be repaired by reducing texture and adding tissue. To achieve this, it is suggested to use native tissues such as cartilage of the nasal septum, because implantation of artificial materials in the nose is associated with many complications. After different incisions and access to the cartilaginous and bony skeleton of the nose, various actions are performed on the structures inside the nose, which varies according to the type of nose.
In general, the complications of Rhinoplasty can be divided into two groups:
1- Damage to nasal function or respiratory obstruction
2- Undesirable changes in the appearance of the nose
If the nasal cavity becomes narrow after surgery and nasal breathing becomes difficult for the patient, the patient is expected to suffer from symptoms such as mouth breathing, dry throat during sleep, vocal breathing, and decreased sense of smell.
Causes of respiratory obstruction following Rhinoplasty include:
1- Do not correct mechanical obstructions during surgery
If there are pre-surgery large nostrils or a deviated septum, the surgeon should try to remove these seemingly minor obstructions as well. Due to the fact that the nose usually shrinks with surgery, if we do not remove the obstacle that already exists and did not create an obstruction, when the nose becomes narrower, this seemingly insignificant obstacle will block the airway.
2- Swelling of the mucosa inside the nose
If the nasal mucosa is swollen due to chronic sinusitis or severe allergies, it may cause respiratory obstruction.
3- Intranasal adhesions
If the surgeon manipulates the nose too much, it will injure the nasal mucosa. If the injured mucosa is in close proximity to each other, it will stick to each other during repair and narrow the way inside the nose.
4- Excessive narrowing of the nostrils
Excessive removal of cartilage from the tip of the nose. This leads to narrowing of the nasal entrance. So far, we have become familiar with the causes of nasal obstruction after surgery, but if the Rhinosurgeon is not experienced enough, in addition to impaired nasal function, there is a possibility that the appearance of the nose will not find the desired shape and become undesirable. Rayan professional team avoids any side effects.
These undesirable forms and the reasons for it are:
1- Asymmetry of the nostrils
The shape and size of the nostrils are affected by the adjacent parts. The condition of the cartilage of the tip of the nose, the front and bottom of the nasal septum, the connection of the nose to the upper lip and the manner and amount of removal from the nostrils all affect the shape of the nostril. If we look at the nostrils of a person who has not had surgery, usually the two nostrils are not exactly the same, but the person who operates after the operation expects that the nostrils are perfectly symmetrical. During the operation, the surgeon should try to correct the asymmetry and correct it as much as possible.
2 – Crooked nose:
In cases where the deviation of the septum is severe, it usually affects the appearance of the nose and the appearance of the nose is not symmetrical and smooth. Complete correction of the deviation of the nasal septum prevents apparent postoperative curvature. If the deviation of the inner septum of the nose is mild before the operation and the surgeon does not correct it during the rhinoplasty, this residual distortion, in addition to causing nasal obstruction, can also cause postoperative distortion.
3- Abnormality of the nose
If the surgeon removes a large amount of nasal tissue, the result is a nose that has a large arch and is so-called slippery, has a large angle, and is so-called pig, and is very narrow in appearance. It is clear that this nose no longer has a normal and natural appearance and gives the person an artificial appearance that is not desirable. Therefore, the surgeon should be careful and avoid making many changes that give the nose an abnormal appearance.
4- Punching or there are depressions on both sides of the nose
The occurrence of this problem, in addition to giving an unfavorable appearance to the nose, causes the sunken part to be an obstacle in the airway and the person suffers from nasal obstruction. The cause of this complication is excessive removal of nasal cartilage.
Naturally, the cartilage that forms the tip of the nose extends to the side of the outer wall of the nostril. The presence of this cartilage causes the negative pressure that enters the different parts of the nose when inhaling to not be able to pull the nasal fins inwards. During surgery, some of this cartilage is usually removed to tighten the tip of the nose. If more than the required amount is removed, the remaining cartilage can no longer withstand the negative pressure of the tail and is pulled in, narrowing the nostrils and making the airway difficult.
- Decreased sense of smell
The terminals of the olfactory nerve are located at the roof of the nasal cavity. This area is not usually manipulated during Rhinoplasty, so there is no chance of damaging the sense of smell after surgery, except in two cases:
Case 1: The person who has undergone surgery already has a large and diffuse nasal polyp and has undergone polypectomy at the same time as the cosmetic operation. Due to the fact that the polyps may be located near the roof of the nasal cavity, removing them and manipulating that part may lead to damage to the olfactory terminals. If a person has a large nasal polyp, it is best to have the polyps removed in a separate operation first and then have a Rhinoplasty a few months later.
Case 2: If, for the reasons I have already mentioned, a person suffers from a degree of nasal obstruction due to Rhinoplasty, the sense of smell will be reduced to the same extent, because in order to be able to feel the smell, the volatile molecules of that substance must be able to pass through the nose Reach the olfactory nerve terminals. If the nose is blocked, these molecules will not be able to enter the nose to stimulate the olfactory nerves.
