Otoplasty

Otoplasty — also known as cosmetic ear surgery — is a procedure to change the shape, position or size of the ears.

Generally, ear correction surgery is safe and most people are happy with the results. However there are risks to consider and it may be expensive.

This reshaping surgery aims to provide a more natural appearance to your ear and can improve self-confidence. It doesn’t impact or improve a person’s ability to hear sounds.

Otoplasty can be done at any age after the ears have reached their full size — usually after age 5 — through adulthood.

Otoplasty is indicated for correction of ears that protrude more than 20 mm and at an angle greater than 35° from the occipital scalp. One or multiple malformed auricular subunits may be managed.

Good candidate for ear surgery

Otoplasty is typically used for ears that, protrude from the head, are larger or smaller than normal,   have an abnormal shape due to injury, trauma, or a structural issue from birth.

Also this surgery has best result in:

  • Ages 5 or older
  • In good overall health, having an underlying condition can increase the risk of complications or affect healing.
  • Nonsmokers, Smoking can decrease blood flow to the area, slowing the healing process.

Otoplasty preparation

Otoplasty procedure

Otoplasty is usually an outpatient procedure. This means you’ll be able to go home the same day.

Before surgery begins, your healthcare provider will give you anesthesia to keep you comfortable. This is done under general anesthesia (where you’re completely asleep) or local anesthesia with sedation (where you’re sleepy, but can still respond to questions).

The specific surgical technique that’s used will depend on the type of otoplasty you’re having. Generally speaking, otoplasty involves:

  • Making an incision, either on the back of your ear or inside the folds of your ear.
  • Manipulating the tissue of the ear, which can include removal of cartilage or skin, folding and shaping of cartilage with permanent stitches, or grafting of cartilage to the ear.
  • Closing the incisions with stitches.

Risks and Complications

Otoplasty is contraindicated in any patient with unrealistic expectations. Patients must receive appropriate preoperative counseling. Discuss existing facial asymmetries, and emphasize that a restoration of anatomic balance to the face is the goal of any surgery. Patients unable or unwilling to cooperate with postoperative care are not candidates for surgery. Advise patients with a history of hypertrophic scarring or keloids that these may occur after otoplasty, possibly distorting an otherwise excellent surgical result. But this surgery has some risks such as:

  • Allergies to tape, suture materials, glues, topical preparations or injected agents
  • Anesthesia risks
  • Asymmetry
  • Bleeding or hematoma
  • Blood clots
  • Change in skin sensation
  • Infection
  • Pain, which may persist
  • Poor wound healing
  • Possibility of revision surgery
  • Skin discoloration/swelling
  • Unfavorable scarring

 

 

Recovery

Full otoplasty recovery usually takes between six and eight weeks. But you’ll probably notice visual improvements in two to three weeks. Keep in mind that healing times vary from person to person.

Your surgeon will want to check on you and make sure your ears are healing well. They’ll likely schedule your first follow-up about one week after your otoplasty surgery. At this point, they’ll remove your dressing (bandages).

Contact your surgeon immediately if you:

  • Develop a fever.
  • Experience excessive bleeding or swelling.
  • Sustain trauma to your surgical site.

Talk to your doctor about when — or if — your stitches will be removed. Some stitches dissolve on their own. Others must be removed in the doctor’s office in the weeks after the procedure.

Ask your doctor when it’s OK to resume daily activities, such as bathing and physical activity.

It takes at least 6 weeks for 50% of the swelling to diminish, 80% within six months, and one year for the final shape to be obtained. However, within 2-3 weeks your ears will look remarkably better. The shape and contour will be readily seen and appreciated after this short period of time.

Things to do to minimize the swelling include: Applying cold packs to both ears for the first 3-4 days, keeping your head elevated as much as possible over the first 2-3 weeks, avoiding bending over or heavy lifting for the first 3 weeks, and avoiding prolonged sun exposure for the first 2-3 months.

Complications of otoplasty have been reported in multiple studies, including hematoma, bleeding, infection, recurrence, skin/wound healing problems, scarring, suture-related problems, pain, and itchiness.

You can wear earphones after the otoplasty if approved by the surgeon. However, you should avoid wearing over-the-ear headphones for at least one month after the ear surgery. You must consult your surgeon before using headphones or earphones or wearing jewelry and other objects on your ear.

Your doctor will test your hearing after your ear has healed. This may be 8 to 12 weeks after surgery. While you are healing, it’s important to avoid getting water in your ear. You will also need to avoid heavy lifting, strenuous exercise, and other activities that may put pressure on your eardrum.

After 24 hours and if you don’t feel sick to your stomach, you can eat soft, regular food. Avoid hot/spicy foods for at least 1 week after surgery.

Hematoma and infection are particularly serious true complications of otoplasty because of the fragile nature of the cartilage. Perichondritis and chondritis are happily extremely rare sequelae as they may provoke veritable disasters.

Although the ears themselves may look quite different after surgery if there is a lot of correction performed with surgery, most patients are surprised that they still look like themselves after surgery. The best part is, they are no longer concerned with the appearance of their ears.

Following up. Otoplasty has a fairly high recurrence rate, but that doesn’t mean you won’t see the results you want. Revisions are common to fine-tune the results of otoplasty, and these are often performed in-office under local anesthetic. Still, 80% of otoplasty procedures have a successful outcome.

Pain from an otoplasty is usually minimal. It is worst in the first 24 hours, but then it rapidly improves. It is impossible to predict a persons pain level. If you have done well with surgery in the past, you will likely be fine after this surgery.

If you are unhappy with otoplasty results and want to have a revision, we will recommend that you wait until your cartilage is completely stabilized. This process typically takes six months to a year. Your surgeon will assess how much you have healed and whether you are ready for revision surgery

You will be under anesthesia during this process. Any scars that form after the procedure will be small and will only show on the backs of the ears, so you — and everyone else — won’t notice them at all. The recovery process after otoplasty is very simple.

Please wear the headband at all times for the first week after surgery. After the first week, please wear your headband at night for the next 5 weeks. You can shower on the second day after your surgery.

You may wash your hair after the bandage has been removed. Be careful not to rub your scalp vigorously. Avoid blow-drying hair until the sensation has returned to normal so that you do not burn your scalp or ears. A headband should be worn at all times to cover the ears with mild pressure.

For the first 1 – 2 weeks after ear pinning surgery, patients should sleep on their back and elevate their head slightly with pillows to help minimize swelling and avoid inadvertently damaging the ears. Additional pillows can be placed around the patient’s side to prevent accidentally turning or rolling while asleep

You will have more than one piece of packing in your ear. If the wound becomes painful, red, inflamed, or starts to discharge, or you experience weakness on one side of your face, or fever, please call the ward or clinic 10. Do not fly, swim or dive for 6-8 weeks after the surgery.
