Breast implants are designed for use in cosmetic enhancement and reconstruction.   According to the American Society of Plastic Surgeons (ASPS), there were approximately 193,000 breast augmentation procedures performed in 2020, making this operation one of the top five cosmetic surgical procedures, after nose reshaping, eyelid surgery, facelift, and liposuction. Breast augmentation, sometimes referred to as a “breast aug” or “boob job” by patients, involves using breast implants or fat transfer to increase the size of your breasts. This procedure can also restore breast volume lost after weight reduction or pregnancy, achieve a more rounded breast shape or improve natural breast size asymmetry.

What breast augmentation surgery can do?

What breast augmentation surgery can’t do?

Breast augmentation does not correct severely drooping breasts. A breast lift may be required along with a breast augmentation for sagging breasts to look fuller and lifted.

Breast lifting can often be done at the same time as your augmentation or may require a separate operation. Your plastic surgeon will assist you in making this decision.

Choosing your implants

The FDA has approved implants for increasing breast size (known as augmentation), for reconstruction after breast cancer surgery or trauma, and to correct developmental defects. The FDA has also approved breast implants to correct or improve the result of a previous surgery.

There are two types of FDA-approved breast implants: saline-filled (saltwater solution) and silicone gel-filled. Both have a silicone outer shell and vary in size, shell thickness, shell surface texture, and shape (contour).

The type of implants most commonly used is silicone gel, but saline implants are also available.

Your surgeon should discuss with you what shape and type of implant is best for you, but below is a rough guide.

Saline-Filled Breast Implants

Saline-filled breast implants contain a silicone outer shell filled with a sterile saltwater (saline) solution. Some are pre-filled and others are filled during the implant operation. Saline-filled implants come in different sizes and have either smooth or textured shells. The FDA approved saline-filled breast implants for breast augmentation in women age 18 or older and for breast reconstruction in women of any age. They are also used in revision surgeries, which correct or improve the result of an original surgery.

Silicone-Filled Breast Implants

Silicone gel-filled breast implants have a silicone outer shell that is filled with silicone gel. They come in different sizes and have either smooth or textured shells.

Silicone gel-filled breast implants are approved for breast augmentation in women age 22 or older and for breast reconstruction in women of any age. They are also used in revision surgeries, which correct or improve the result of an original surgery.

Pros and cons of Silicone gel implants and Saline solution implants:

Pros: Generally less likely to wrinkle than saline implants. The soft type can give a natural feel. Polyurethane-coated ones are claimed to reduce the risk of capsular contracture and the implant rotating

Cons: If a soft gel implant ruptures, silicone could spread into your breast and cause siliconomas. This is much less of an issue if cohesive implants are used, but this firmer gel may not feel as natural. Polyurethane-coated ones may cause a temporary skin reaction.

Pros: If the implant were to rupture, the saline solution would be safely absorbed or passed out of your body.

Cons: More likely to rupture or deflate earlier than silicone, as they tend to slowly deflate over time. More prone to wrinkling or folding. May feel less natural than soft silicone.

 

Where to place them

The implants can either be placed between the breast and the chest muscle or behind the muscle. You’ll need to decide with your surgeon where is best for you, but here’s a rough guide.

Between the breast and the muscle

Risks of breast augmentation

The decision to have plastic surgery is extremely personal and you will have to weigh the potential benefits in achieving your goals with the risks and potential complications of breast augmentation. Only you can make that decision for yourself.

You will be asked to sign consent forms to ensure that you fully understand the procedure and any risks and potential complications.

Possible breast augmentation surgery risks include:

  • Anesthesia risks
  • Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) or other very rare cancers in the capsule around the breast, such as breast implant-associated squamous cell carcinoma (BIA-SCC)
  • Bleeding
  • Changes in nipple or breast sensation
  • Fluid accumulation (seroma)
  • Formation of tight scar tissue around the implant (capsular contracture)
  • Hematoma
  • Implant leakage or rupture
  • Infection
  • Persistent pain
  • Poor scarring
  • Possibility of revision surgery
  • Wrinkling of the skin over the implant
  • Wrong or faulty position of the implant

How long do breast implants last?

Breast implants will not last a lifetime. They are likely to need replacing at some point.

Some women may need further surgery after about 10 years, either because of problems with the implants or because their breasts have changed around the implants.

Recovery

It can take a few weeks to fully recover from breast implant surgery. You may need to take a week or two off work and shouldn’t drive for at least one week.

Some surgeons recommend wearing a sports bra 24 hours a day for up to three months after breast surgery.

You’d need to avoid heavy lifting or strenuous exercise for at least a month, and keep your breast scars out of direct sunlight for about a year.

After a week or two: Stitches would be removed (unless you had dissolvable stitches).

Within six weeks: You’d usually be able to return to most of your normal activities.

After a few months: Your breasts would usually start to look and feel more natural. You may be able to stop wearing your sports bra.

Other important considerations

  • Breast implants are not guaranteed to last a lifetime and future surgery may be required to replace one or both implants
  • Breast augmentation requires regular examinations of your breast health and to evaluate the condition of your breast implants
  • The FDA recommends that you get your breast implants screened – using an MRI or ultrasound – five to six years after silicone implant placement and every two to three years thereafter
  • Pregnancy, weight loss and menopause may influence the appearance of augmented breasts over the course of your lifetime

Even if you do not have concerns about the condition of your implants, it is important to go for your routine exams and screenings, based on the FDA-recommended timeline. Routine screenings can verify your implant is intact and identify complications such as implant rupture or silicone leakage. Although implant rupture can cause various symptoms, some women with ruptured implants experience no symptoms, which is why routine screenings are critical.

This is one of the most frequently asked questions by women considering this surgery. Many studies have shown that women who get implants are not at an increased risk for getting breast cancer. In addition, implants do not delay the detection of breast cancer. Numerous studies, as well as evidence from the National Cancer Institute, show that having implants does not hinder the patient or their physician in finding breast tumors. Finally, breast implant patients who do get breast cancer have the same chance of obtaining remission, or being free of cancer for 5 years, as women without implants.

Many women in their 40s and 50s have undergone breast augmentation surgery with good results. The most common patient for this surgery is a woman in her 30s with two children who has been considering this operation for many years. Whatever your stage in life, we encourage you to schedule a consultation to find the best path toward your cosmetic goals.

Pregnancy will change the size and shape of a woman’s breasts whether she has implants or not. These changes may adversely affect the cosmetic appearance of any augmented breast. It is usually recommended that pregnancy be postponed until 6 months after the surgery. Breast-feeding with implants is still possible and has been shown to be safe for the baby and the mother.

Women with sagging breasts, which is called “ptosis”, can also receive breast implants. Depending on how much the breast sags, an additional surgery may also be necessary. For breasts with nipples that droop below the lower crease of the breast, a breast lift surgery, or mastoplexy, is often required as well. This surgery will also add scars all the way around the nipple-areola unlike most breast augmentation surgeries. Without this additional surgery to remove excess skin, placement of the implant may not adequately correct the sagging.

This is often one of the most difficult decisions, and for many reasons, has the most potential for disappointment. During your consultation, your plastic surgeon will work with you to identify what size of breast you desire from augmentation. “Breast size” as measured by bra-size is variable and is often not a good way to measure final size. As a general rule, every 125-150cc of implant volume equals an increase in a single cup size. However, every patient’s body is different. By working with your surgeon and their clinical team, you can set obtainable and healthy expectations together. It is helpful for you to bring in a bra the size you would like to be to help in finding the right sized implant for you.
